DENVER (CBS4) – On Friday, the State of Colorado announced the first of five million dollar winners in the Colorado Comeback Cash draw. The state picked the winner from the 2.9 million Coloradans who received at least one dose of their COVID-19 vaccines.
On Thursday, the state’s health department explained how eligible Coloradians can verify their enrollment.
There are two ways. The first is a public website portal.
“It’s not a perfect portal,” said Health Roth, chief of the immunization branch of the Division of Disease Control and Public Health Response.
Roth said, “You have to have the data that is exactly what the health care provider said. “
And some health care providers don’t provide a phone number or email. This information is also necessary.
“The great thing I want people to understand is just because they can’t find their case using this public portal doesn’t mean their case doesn’t exist,” Roth said.

A second option for contacting the state program is to simply send an email to the CIIS help desk.

Requirements for retrieving vaccination records:
• Patient information (last name, first name, date of birth and gender) must match the information on the patient’s CIIS profile.
o Note: If you or the minor you are requesting records for has changed their name, the patient profile may need to be updated in order to access records.
• There must be a cell phone and / or email address on the CIIS patient profile to which you have access, and it must match what entered in the search.
o Note: the mobile phone must be able to receive text messages (SMS).
• If you cannot access immunization records through the public portal, please contact the CIIS Helpdesk at [email protected] to update patient information.
• Note: the CIIS assistance service must verify your identity before giving access to the requested files. If they are unable to verify your identity based on what is currently in CIIS, you will be prompted to contact your healthcare provider to update the information as needed.
